Patients often present for corrective surgery for breast implants with us weekly. They present with a myriad of different problems, which can include severe encapsulation, capsular contracture, double-bubble deformity, visibility, palpability of saline implants, ptosis, bottoming out, ruptured implants, malposition of implants and severe disfigurement due to improperly placed implants.
Patients who present for corrective surg
ery for their breast implants are often unhappy with the appearance of their breasts or they are in severe pain. Pain level can be considered via the Baker classification. Baker 1 is no pain whatsoever in a soft, supple breast; Baker 2 is palpable contracture, Baker 3 is palpable and visible; and Baker 4, the worst, is hardening with severe thickening of the scar tissue causing pain, tenderness and disfigurement to the breasts. The breasts can be cold as well.
In any case, the corrective surgeries require multiple approaches, which include open capsulectomies with removal and release of scar tissue; open capsulotomies with simple circumferential opening and releasing of the tissue expansion by increasing volume for the implants; releasing pressure within the external chest cavity; repositioning of implants and change of the implants (moderate profiles can be replaced with high profiles), which may reduce some of the lateral fullness of the breasts; formal mastopexies; inframammary tightening procedures; and capsulorrhaphies with internal tightening of the capsule can also be done in order to elevate the inframammary folds.

Double-bubble deformities are corrected by releasing the inferior capsule, removing scar tissue if necessary and often by performing a formal mastopexy using the anchor scar or the Wise-pattern technique.
